They were symptoms straight out of a textbook – shortness of breath, sleeping on pillows, unable to walk more than a block or up a flight of stairs, chest pain. Congestive heart failure is a parasite: slow growing, insidious, and debilitating. The backup of blood from a slowly failing heart results in fluid congestion in the lungs and legs, causing the constellation of symptoms in the elderly woman before me. I was down in the ED, stumbling through questions. It was my first time taking a history on a real patient, and my lack confidence and discomfort were apparent. Half of the sounds coming out of my mouth were ‘ums’ as I grasped for words. I awkwardly leaned over her as I listened to her heart. To be honest, this was mostly performative – I hardly even knew what a failing heart was supposed to sound like. And to put an elegant end to it all, I pressed down on her puffy legs, removing my hands but seeing the impression of my thumbs on her shins remain. Pitting edema. “Does that hurt?” I asked. Her brows furrowed as she stared at me, but she was calm and listless in her answer – “Of course.” My inexperience was showing. I felt dumb for even asking. After I had finished, my resident took over and filled in the gaps of the numerous things I forgot to ask. His questions were fluid and his delivery seamless. Good history takers are also good diagnosticians. Their minds are already thinking of the ten most likely diagnoses based on one symptom, and they narrow this list by asking questions that can rule each diagnosis in or out. The best history takers, however, are the ones that listen to the patient. After all, a differential diagnosis can’t be built around symptoms that aren’t reported. The guilt of having put her through the distress of my interrogation and exam weighed on me as we left. I think this is every medical student’s fear – that somehow, patients are worse off because we are involved in their care. There’s this undeniable feeling that these patients – real people – are individuals we’re practicing on. I was one of her providers, but in that moment, I wasn’t able to provide her with anything. —

-G ET-T OG E T H E R- Lichens are not a single organism! They represent a symbiosis of at least one fungus which is called the mycobiont and at least one algae which is called the photobiont. The algae can be an eukaryotic green algae (chlorolichen) or a prokaryotic cyanobacterium (cyanolichen). Both perform photosynthesis and produce sugar alcohols that are taken up by the mycobiont. The mycobiont provides a home with it's dense matrix of hyphae, perfectly arranges the photobiont towards sunlight, protects it with pigments from uv radiation and provides extra water. This symbiosis goes so far that the mycobiont even controls the morphology of its photobiont. Today I show you a microscopic cross section of the cyanolichen Leptogium burgessi which contains the cyanobacterium Nostoc as photobionts that are blueish in the image. If the photobiont is taken off the lichen and brought in culture, the morphology of the Nostoc photobiont changes extremely as you can see in the second image. The third image shows the intact gelatinous lichen Leptogium burgessi that can be found world wide. The routine of morning rounds is such that any minute change is wildly apparent and a stark reminder of how quickly things can change in the usual slow hum of the inpatient world. We were on our way to see a patient who had been admitted a few days prior with the flu complicated by her failing kidneys and lungs. Further complicating things, she was found to have multiple other serious infections and didn’t speak any English. When we walked in the room that morning, having already donned our masks and gowns, it was clear something had changed. She was delirious and confused – unable to answer our basic questions of who or where she was, asked through the crackly voice of our translator who was on speakerphone. As we exited the room, the resident began rattling off a list of things that could have caused this acute change in mental status, saving the most serious for last – the possibility that one of her infections had spread to her brain. We headed back to the workroom to get on with our tasks – putting in orders, calling consults, and following up on lab results. This patient’s delirium was still on my mind, specifically the possibility of this brain infection. I recalled from class that antiviral therapy should be started immediately to improve mortality outcomes, but didn’t see any orders in. I suggested this to the resident. He stopped and looked at me sternly. For a second, I thought he was going to lecture me for questioning his medical decision making. However, his look softened as he said, “That’s a great idea. I want you to pick up this patient.” I sat there, proud of myself that I had, for the first time, suggested something that changed how a patient was managed. But part of me was trying to grasp what it would mean to look after her. The weight of her becoming MY responsibility became apparent all too quickly. A few minutes later, I broke the silence again. “… I’m afraid she’s going to die.” I think my honesty surprised him. In medicine, emotional guarding as a defense mechanism is virtually an expectation. In that rare moment of earnestness, I think he let his guard down for the first time in a while. “Me too.” —
